Informed Consent for Telemedicine Services

Telemedicine involves the use of electronic communications to enable health care providers at different locations to share individual patient medical information for the purpose of improving patient care.

Providers may include primary care practitioners, specialists, and/or subspecialists. The information may be used for diagnosis, therapy, follow-up and/or education, and may include any of the following:

  • Patient medical records
  • Medical images
  • Live two-way audio and video
  • Output data from medical devices and sound and video files

Electronic systems used will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional or unintentional corruption.

Expected Benefits:

  • Improved access to medical care by enabling a patient to remain in his/her physicians office (or at a remote site) while the physician obtains test results and consults from healthcare practitioners at distant/other sites.
  • More efficient medical evaluation and management.
  • Obtaining expertise of a distant specialist.

Possible Risks: As with any medical procedure, there are potential risks associated with the use of telemedicine. These risks include, but may not be limited to:

  • In rare cases, information transmitted may not be sufficient (e.g. poor resolution of images) to allow for appropriate medical decision making by the physician and consultant(s);
  • Delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment;
  • In very rare instances, security protocols could fail, causing a breach of privacy of personal medical information;
  • In rare cases, a lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors;

By signing this form, I understand the following:

  1. I understand that the laws that protect privacy and the confidentiality of medical information also apply to telemedicine, and that no information obtained in the use of telemedicine which identifies me will be disclosed to researchers or other entities without my consent.
  2. I understand that I have the right to withhold or withdraw my consent to the use of telemedicine in the course of my care at any time, without affecting my right to future care or treatment.
  3. I understand that I have the right to inspect all information obtained and recorded in the course of a telemedicine interaction, and may receive copies of this information for a reasonable fee.
  4. I understand that a variety of alternative methods of medical care may be available to me, and that I may choose one or more of these at any time. My physician has explained the alternatives to my satisfaction.
  5. I understand that telemedicine may involve electronic communication of my personal medical information to other medical practitioners who may be located in other areas, including out of state.
  6. I understand that it is my duty to inform my physician of electronic interactions regarding my care that I may have with other healthcare providers.
  7. I understand that I may expect the anticipated benefits from the use of telemedicine in my care, but that no results can be guaranteed or assured.

Patient Consent To The Use of Telemedicine

I have read and understand the information provided above regarding telemedicine, have discussed it with my physician or such assistants as may be designated, and all of my questions have been answered to my satisfaction. I hereby give my informed consent for the use of telemedicine in my medical care.

I hereby authorize Regency Physicians and a member of his or her staff to use telemedicine in the course of my diagnosis and treatment.

CONSENT FOR TREATMENT, ASSIGNMENT OF BENEFITS, AND FINANCIAL POLICIES

I. Consent for treatment: I authorize Regency Skin Institute PLLC; DBA Regency Dermatology and/or Mussman PLLC; DBA Regency Plastic Surgery its agents, Betty Hinderks Davis, MD Jason Mussman, MD, Benjamin Jones , MD, Brent Schultz, MD,Zoe MacIsaac, MD, Stephanie Spahr, PA-C & Brittany Silverman, PA-C to render treatment to me/my dependents for dermatological and medical/surgical care.

II. Assignment of Benefits/Release of medical information: I request that payment for authorized Medicare or other applicable private insurance benefits be paid directly to Regency Skin Institute PLLC; DBA Regency Dermatology and/or Mussman PLLC; DBA Regency Plastic Surgery for services provided under their care. I also authorize Regency Skin Institute PLLC; DBA Regency Dermatology and/or Mussman PLLC; DBA Regency Plastic Surgery to release necessary medical information to my insurance company, its agents, or any third party in order to determine pa

**Once your deductible is met, many insurance companies still do not pay 100% of the healthcare cost. If that is the case you will have a copay or co-insurance, which is a partial payment required by you in addition to what the insurance company will pay. It can be from 10-50% of the allowed amount until you have accumulated enough medical bills to meet your yearly out-of-pocket maximum.

***This is an estimated portion that is due. Unfortunately we don’t know exactly what your insurance will cover or what you will be billed until your claim is processed.

V. Referrals/Authorization: I understand that if my insurance company requires a referral, I am responsible for obtaining a referral prior to my visit. If I do not have a referral at the time of service, no services will be rendered until I obtain a referral or sign a waiver of financial responsibility. Payment in full is required at the time of service.

VI. Missed Appointments: Our office requires 24 hour notice for cancellations. Patients who fail to show to the initial visit will be forgiven, If a patient chooses to schedule a second appointment we will place a credit card on file and If the second appointment is missed we will process the credit card on file for $75.00 (non refundable). If a third no show or cancellation/reschedule with no 24 hour notice should occur the patient will be charged $90.00. If a fourth should occur the patient will be charged $100.00 and be dismissed from Regency Skin Institute PLLC; DBA Regency Dermatology and/or Mussman PLLC; DBA Regency Plastic Surgery.

Each time you visit our offices, we record your symptoms, physical examination, test results, diagnosis, and treatment. YOUR RIGHTS

Although your paper chart belongs to our practice, the information contained in the chart is yours. You have the right to: inspect your records, obtain a copy of your chart for a small fee, correct your records, and tell us not to release your information.

OUR RESPONSIBILITIES

We are required to: maintain the privacy of your health information; send needed health information to other medical providers, and release information to insurance companies, certain government agencies, and others. We may be required to release some information, even without your permission.

OTHER NOTICES

We may leave a message at your home, at your business, on your answering machine or on your voicemail. We may mail you a postcard or other written notices. We may need to disclose your information to your family members or other people helping with your care. In doing so, we will use our best judgment. We may disclose information to others as required by law or if subpoenaed. If you were injured on the job, we will need to disclose your health information to your workers compensation insurance company. We may, from time to time, update these policies.

A physician must notify a patient that the physician has a direct financial interest in a separate diagnostic or treatment agency to which the physician is referring the patient and/or in the non- routine goods or services being prescribed by the physician, and whether these are available elsewhere on a competitive basis. A.R.S. §32-1401(27)(gg). (I/We) support this law, because it helps patients make reasoned financial decisions concerning their medical care.

Dermatology

Betty A. Hinderks Davis, MD Board-Certified Dermatologist

Dr. Betty Hinderks Davis is a nationally recognized board-certified dermatologist and a fellowship trained Mohs Surgeon who has practiced in the greater Phoenix area since 2005. She has advanced the field of Dermatology and Mohs Surgery through her numerous scientific and educational contributions. While an expert in skin cancer diagnosis and treatment, she is passionate about skin cancer prevention and early detection as well as working in conjunction with her patients to achieve optimal skin health.

Dr. Davis graduated as an Honors Scholar from the University of Missouri-Columbia (UMC) majoring in Biochemistry and completed her medical school training at the UMC graduating Cum Laude. Her residency training in dermatology was then completed at the UMC. Her surgical fellowship took her to Birmingham, Alabama where she completed a Mohs and Dermatologic Procedural Fellowship with Dr. Gary Monheit. She also received extensive cosmetic training during this time. Dr. Davis’ fellowship culminated with the receipt of the Mohs College Theodore Tromovich Award for research done on periorbital tumors in 2004, which is the highest national fellow award presented by the Mohs College. In April 2017, she was also awarded the Outstanding Young Physician Honor for being distinguished in and contributing to the advancement of dermatology as a medical profession by the University of Missouri.

Dr. Davis is passionate about the patient experience and was the national recipient of the “Top Performer Clinic — Overall Patient Experience” in 2013 from Health Stream. Dr. Davis believes that great health care begins with a patient-centric team-based approach and works to ensure patients have the right care when needed and early diagnosis and treatment and cultivates an environment where patients are active in their care.

Benjamin B. Jones, MD

Dr. Benjamin B. Jones graduated from Brigham Young University with a Bachelor of Arts Degree in the discipline of History as well as graduating with University Honors, the highest academic distinction awarded to undergraduate students. While at BYU, he spent a semester abroad at Kings College, Cambridge in the United Kingdom, where he participated in research evaluating spinal cord injury repair. Also during his time at BYU, with the mentorship of Dr. James Dee Higley, Dr. Jones published research on the mother-infant relationship of Rhesus Macaque monkeys and their effects on the HPA axis.

Dr. Jones earned his medical degree from the University of Utah School of Medicine. While completing his training, he was awarded a summer research fellowship where he worked on molecular mechanisms of malignant melanoma at the Huntsman Cancer Institute under the direction of Dr. Douglas Grossman. He was later awarded funding from the National Psoriasis Foundation and became a Psoriasis Research Fellow in the department of dermatology at the University of Utah, where both Drs. Kristina Callis-Duffin and Gerald Krueger served as mentors. His work included data collection for the Psoriasis Longitudinal Assessment and Registry (PSOLAR), collaboration with the National Institute of Health on work pertaining to the Human Microbiome, research on immunobullous disease affecting mucosal tissue, and development of evidence-based survey data collection.

Dr. Jones completed his Medicine Internship at Legacy Emanuel and Good Samaritan hospitals in Portland, Oregon. He completed dermatology residency at the University of Utah where he served as a chief resident his final year of training.

Dr. Jones greatly enjoys teaching and helped restructure the didactics curriculum for his fellow residents. He also continued his work on immunobullous disease and its effects on esophageal tissue, and is hopeful for the positive impact this data will have for patients.

Dr. Jones is a fellow of the American Academy of Dermatology (AAD) and of the American Society for Dermatologic Surgery (ASDS). He is incredibly excited to be joining Regency as a general dermatologist. His areas of interest include skin cancer treatment and prevention, immunobullous disease, papulosquamous disease, cutaneous T-Cell Lymphoma, and cosmetics.

Stephanie Spahr, PA-C

Stephanie Spahr was born and raised in Tucson, Arizona. She received her Bachelor of Arts in Chemistry at Grinnell College in Iowa and went on to graduate school at Midwestern University in Glendale. She spent time as a CNA in nursing homes and hospice and was a PT technician prior to attending PA school.

Her three greatest accomplishments include serving on a medical mission trip in Panama, being captain of her collegiate golf team during the first time in history the team made it to Nationals, and running a marathon. She has always had a passion for dermatology and looks forward to continuing to care for patients and practicing in the field.

  • Master of Medical Science in Physician Assistant Studies- Midwestern University

Brittany Silverman, PA-C

Brittany Silverman is an Arizona native and nationally certified as a Physician Assistant. She completed her undergraduate education at the University of Arizona, where she graduated Cum Laude with a Bachelor of Science in Physiology. She then earned her Master’s degree at Hofstra University in Long Island, New York. As a graduate student, she received clinical dermatology training at a private practice in her hometown of Tucson, Arizona. After earning a perfect score on the Dermatology section of her licensing exam, Brittany completed additional training from the American Academy of Dermatology and became a proud Diplomate Fellow through the Society of Dermatology Physician Assistants.

Brittany has a strong passion for Dermatology which began as a teenager. She strives to treat all her patients with a high degree of understanding and compassion. Her mission is to provide excellent patient-centered care and to help her patients feel comfortable and confident in their own skin.

In her free time, Brittany enjoys spending time with her family and friends. She lives a very active lifestyle and loves hiking, traveling, running, cycling, and swimming.

Chen Wang, MD

Chen Wang, M.D. is a board-certified pathologist and board certified dermatopathologist. Dr. Wang received her M.D. degree from China Medical University and pursued postgraduate biomedical research training at Massachusetts General Hospital and Harvard Medical School.

Dr. Wang completed her residency training in pathology at the St. Barnabas Medical Center and her fellowship training in gynecology and breast pathology and cytology at Brown University School of Medicine. She also served as a clinical instructor of pathology at Brown Medical School during her fellowship in 2002.

Dr. Wang practiced as a general pathologist at Monongalia General Hospital and Ohio Valley Medical Center from 2002, until 2004 when she began her dermatopathology fellowship training at Tulane University School of Medicine.

Hand Surgery

Brent Schultz, MD

Dr. Schultz appreciates both the art and science of surgery. He graduated from the University of California at Davis with honors in both art and genetics. He also played college football.

At Yale Medical School, Dr. Schultz earned two advanced degrees and a fully-funded fellowship through the Doris Duke Research Foundation. He graduated with the highest honors in medicine having been elected by the faculty to possess the best qualifications to be an outstanding physician and wrote the most outstanding research thesis for the graduating class. His research in vascular biology has resulted in two patents.

Dr. Schultz completed a residency in plastic and reconstructive surgery at the University of Washington. He then further specialized in hand and microsurgery from the University of Pittsburgh Medical Center, a combined orthopedic and plastic surgery fellowship.

He has spent multiple years at a high volume, high acuity hand and craniofacial trauma center in Phoenix, Arizona servicing multiple level one trauma hospitals. For the last two years, he has been performing cosmetic surgery, as well as advanced breast, body and hand reconstruction.

In addition to hand and plastic surgery, Dr Schultz performs minimally invasive wrist surgery for arthritis and injury, as well as the endoscopic carpal tunnel release. He has expertise in upper extremity trauma from the elbow to fingers and performs microsurgical operations for arm and hand reanimation following nerve injuries. In addition, Dr Schultz uses his microsurgical expertise for all forms of advanced breast reconstruction. He is skilled in multiple cosmetic procedures from facial rejuvenation to body and breast recontouring/augmentation.

He is a board certified in plastic and hand surgery and has published multiple papers in peer reviewed journals on these subjects.

Plastic Surgery

Zoe MacIsaac, MD

Zoe MacIsaac, MD, attended the University of Virginia College of Arts and Sciences to earn her undergraduate degree in biology, followed by completing her medical degree at the University of Virginia School of Medicine. She then continued her education at the University of Pittsburgh Medical Center, spending two years in the Craniofacial Biology Laboratory at the Children’s Hospital of Pittsburgh. During that time she performed basic science research on bone tissue regeneration for calvarial defects, in addition to clinical research. Dr. MacIsaac’s work has been presented at numerous regional, national and international meetings and she has won awards including the Kawamoto Best Paper at the American Society of Craniofacial Surgeons, and the AO North America Best Overall Paper Award. Dr. MacIsaac has co-authored book chapters in “Operative Otolaryngology” and in “Aesthetic Surgery for Men”.

Dr. MacIsaac completed six years of integrated plastic surgery training at the University of Pittsburgh Medical Center (UPMC), consistently ranked by Doximetry as one of the top three training programs in the United States. She sought to attend UPMC to learn from world class faculty, continue to enjoy a wide range of research opportunities, and to experience the breadth of cases in plastic surgery, from microsurgical replantation and breast reconstruction, to hand and face trauma at a multiple Level 1 centers and a magnet children’s hospital, to ample aesthetic procedures.

Dr. MacIsaac enjoys the breadth of plastic surgery. She is dedicated patient safety and to restoring form and function, with commitment to achieving natural appearing, long-lasting results.

Her pecial interests include: facial aesthetics (including rejuvenation and rhinoplasty), facial trauma (and general reconstruction), breast reconstruction, skin cancer reconstruction, fat grafting for breast and face.

Jason Mussman, MD Board Certified Plastic and Reconstructive Surgeon

Dr. Mussman is one of Phoenix / Scottsdale’s most experienced and well respected plastic surgeons.
